Reference Group and Consortium

Bayside Community Options Inc, Sandgate and Bracken Ridge Action Group (SANDBAG) and Redcliffe Community Association (RCA) are working together as a reference group and consortium partners to plan and implement programs for, and with, people who have severe mental illness.   

Bayside Community Options delivers a case management model of service specializing in Home and Community Care (HACC) for eligible older and younger consumers with complex needs. A range of interacting physical, medical, social and emotional needs cannot be adequately met by mainstream HACC services. A diagnosis of severe mental illness applies to about 10% of the approximately 300 people on our current case load. BCO provides assessment and case management involving consumers in this process. We work collaboratively with other agencies, allied health teams and clinicians for referral purposes, and to provide flexible continuity of care and linkages with the community to achieve consumer preferred outcomes. Where relevant, we actively engage families and carers in these processes. Rather than encourage dependency Support Workers coach and encourage people to participate in activities of daily living. Support Workers have regular rosters to provide services to the same people once a week or fortnightly and therefore build strong, trusting relationships with consumers. BCO also provides a culturally appropriate outreach program in the local Indigenous community.

The community centre at Sandgate known as Sandgate and Bracken Ridge Action Group (SANDBAG) began in the 1980’s with local residents working together to respond to issues of local concern. The Shorncliffe train line from Brisbane CBD and beyond sees an increasing number of persons arriving to seek shelter and a more relaxed lifestyle in these bayside suburbs. This is attractive to people who are wealthy as well as people who are financially disadvantaged and homeless. SANDBAG records 4,400 points of contact every quarter and engages with persons with a history of mental illness. SANDBAG provides personal support to those who participate in aspects of the Drop-In Program and the centre also conducts the Sandgate Employment Pathways Program (SEPP). At least 80% of this work engages on average10-14 people per week with a history of mental illness.

Redcliffe Community Association (RCA) operates a Mental Health Outreach Support Program through Sunshine Place. This is an in-home support program for those aged 13-65 diagnosed with a mental illness. The focus is to assist people (including families) experiencing barriers, as a result of their mental health issues, to access opportunities and to develop positive pathways to their recovery. RCA also has a centre based program specifically for people recovering from mental health illnesses.

Sunshine Place offers a Personal Enrichment Program including mental health information, individual support, access to computer facilities, recovery sessions, skill workshops, social outings and recreational activities. This program incorporates a consumer advisory committee to assist with articulation of activities, planning and writing the bi-monthly newsletter.
