Information for agencies and health professionals

Bayside Community Options Inc provides personalized, responsive and reliable care. We focus our support on enhancing people’s strengths and maintaining their independence and community connection. As a Community Options Program, BCO delivers assessment and case management with consumer, carer and family input and specializes in flexible services tailored to people with complex and multiple needs. 

Some of our services include:

  • PERSONAL CARE
    assistance with showering, hair washing and dressing or just being around while this happens to give a sense of security

  • RESPITE CARE
    keeping people company or helping them to participate in activities at home or in the community while families or other carers have some “time out”

  • SOCIAL SUPPORT
    preventing isolation by linking people with community activities – for example shopping, banking, libraries, clubs, coffee shops, programs at the local community centre

  • DOMESTIC ASSISTANCE
    assisting with basic domestic tasks such as vacuuming, mopping, cleaning the bathroom; encouraging and skilling people to do those tasks they can do themselves

  • HOME MAINTENANCE
    mowing lawns once a month and occasional rubbish removal and window cleaning. We refer repairs to Sandgate Home Assist Secure.

  • TRANSPORT
    being driven in a BCO vehicle or taxi to and from medical appointments. This is not an alternative taxi service but provided for people with assessed need. We also accompany people using public transport until they are more self sufficient.

  • OTHER FOOD SERVICES
    preparing meals and shopping lists together

  • GOODS AND EQUIPMENT
    as assessed and recommended by occupational therapist. This would include purchase or hire of items such as medical alerts, wheelie walkers, continence pads, shower chairs, wheel chairs or walking sticks.

  • OTHER PROGRAMS AND SERVICES

We aim to have the same Support Worker provide services at the same time with the same consumers. This helps build caring and trusting relationships.

Referrals

Referrals from a general practitioner, Aged Care Assessment Team, discharge nurse or facilitator, Transition Care provider, Mental Health team or case managers, other health professionals as well as from family members, other carers or neighbors and self referrals are accepted.
